Sensitive intervention through Posyandu cadre training and assistance for mothers under five to reduce stunting in Genaharjo Village, Tuban

Abstract

Nutrition problems are still a major public health issue, especially stunting which has a long-term impact on children's growth
and development. Based on a report by the Tuban Regency Health Office in 2022, the prevalence of stunting in the work area
of the Wire Health Center reached 24.43%. Genaharjo Village is the focus of attention because the number of stunted children
under five was recorded at 59 children in 2022, decreasing to 40 in 2023, but increasing again to 45 (14%) in 2024. In addition,
66.7% of pregnant women are at high risk of Chronic Energy Deficiency (CED). This condition is still far from the national
target of reducing stunting by 14% and wasting by 7% by 2024. Community service activities were carried out in Genaharjo
Village in August-September 2025 through the training of cadres integrated service post and the assistance of mothers under
five. Evaluation was carried out by pre-test and post-test of knowledge, observation of cadre skills in anthropometry,
assessment of the skills of mothers under five in food processing, and urine examination of toddlers. The results showed an
increase in the knowledge of cadres from an average of 9.25 to 11.25, as well as an increase in the knowledge of mothers under
five from 9.6 to 11.1. The cadres integrated service post are able to properly prepare tools, measurement techniques, recording,
and communication. Mothers under five are skilled in menu planning, ingredient selection, processing, presentation, nutritional
assessment, and education, producing 20 healthy menus based on local ingredients. Rapid test pyridinium crosslinks
examination with 20 samples in the form of stunted toddler urine under five showed that 13 children were examined (2 positive,
11 negative), while 7 were not examined. Increasing the knowledge and skills of cadres and mothers of toddlers proves that
community-based education is effective as a stunting prevention strategy. However, the presence of positive urine results
indicates that nutritional interventions such as providing additional food are not optimal, so a comprehensive approach is
needed that includes nutrition education, health monitoring, and sanitation improvements to make stunting prevention more
sustainable
